Battalion: 1st Battalion Sherwood Foresters (Notts & Derbys) Regiment.

He was born on 23rd September 1892. Son of Sam and Martha Hayes, of Chatsworth Street, Sutton-in-Ashfield, Nottinghamshire. He enlisted in January 1915 and served in Gallipoli, where he suffered from frostbite and blood poisoning. He was invalided to Manchester Infirmary, England. He returned to France on 2nd June 1916 and was killed in action on 5th July 1916 in the attack on the German positions at La Boisselle, France, during the Battle of the Somme.
