Born at Leeds, Yorkshire. Son of Robert and Sarah Forth, of 12, Cherry Tree Row, Marecroft, Worksop, Nottinghamshire. He enlisted in November 1914 age 16 and died 27th April 1916 of wounds the previous day during the advance along road Northumberland Road to Mount Street Bridge, Dublin, during the Irish Rebellion,
