British War and Victory MedalBorn in Derby. Son of Isaac And Annie Farnsworth, of 12, Waterway Street West, Nottingham. Lived at 74 Mill Street, Ilkeston, Derbyshire. He enlisted at Ilkeston 25th May 1915, went to France 6th March 1916, having been appointed to Lance Corporal 5th July 1915. He was wounded on 18th May 1916 with gunshot wounds to his face and was treated at 133rd Field Ambulance. He rejoined his battalion 26th May 1916 and suffered gunshot wounds to his right leg 3rd July 1916, he died of his wounds at No. 33 Casualty Clearing Station, Bethune, France, on the same day at 7 pm.
