Born and lived at Kidderminster, Worcestershire. Son of William John and Sarah Ann Cook, of 130, Park Street, Kidderminster. He was an office clerk prior to gaining a commission with the 5th Battalion Sherwood Foresters (Notts & Derbys) Regiment, 31st October 1917. He died of wounds on 16th April 1918, aged 26.
