Born 10th February 1898 at Upper Warlingham, Surrey. Son of Frank and Sybil Cook, of Endcliffe, Caterham Valley, Surrey. He was educated at Marlborough College and was a member of the Nottingham University Training Corps. He was commissioned Second Lieutenant Sherwood Foresters (Notts & Derbys) Regiment, 1st March 1917 and joined the 9th Battalion Sherwood Foresters (Notts & Derbys) Regiment at Bapaume, France 25th April 1917. He was killed in action, 9th June 1917, aged 19, in the attack at Wytschaete, near Ypres, Belgium.
