Born and lived at Chesterfield, Derbyshire. Son of William and Helen Cook, of 21 Derby Road, Chesterfield. He was an errand boy prior to enlisting at Chesterfield. He went to France 8th May 1915 and was killed in action, 22nd September 1918, during a German attack and British counter-attack on the lines at Pontruet, France.

Died: 22nd September 1918.
