Battalion : 2nd Battalion Sherwood Foresters (Notts & Derbys) Regiment.

Born at Cawthorpe, Lincolnshire. Son of John Thomas and Dinah Chambers, of 11, Stanley Street, Bourne, Lincolnshire. He was a coal miner prior to enlisting on 11th August 1914 at Nottingham. He went to France on 27th December 1914 and suffered a gunshot wound to his head, 9th August 1915. He was admitted to No. 25 general Hospital, Etaples, France, on 10th August 1915 and evacuated to England on Hospital Ship, Dieppe, on 17th August 1915. He returned to France on 9th December 1915 and suffered gunshot wounds to his left thigh between the 13th and 15th September 1916. He died, 20th September 1916, aged 26, at No. 11 Stationary Hospital, Rouen, France after having his leg amputated.
