Born and lived at Tottington, Lancashire. Son of Samuel and Sarah Jane Chadwick, of 16, Holcombe Road, Tottington. He was dyer’s labourer prior to enlisting at Tottington. He was killed in action, 20th November 1917, age 23, in the attack at Beaucamps, near Ribecourt, France.
