Battalion: 2/5th Battalion Sherwood Foresters (Notts & Derbys) Regiment.

Son of Alfred and Sarah Blockley, of Potter Street, Melbourne, Derbyshire. He was a gardener prior to enlisting at Derby.  He was killed in action, 4th March 1918, aged 25, whilst with the Battalion at Noreuil, France.
