British 1914 Star, British War and Victory MedalBorn at Old Normanton, Derbyshire and lived at 27 Trafalgar Street, Derby. Son of Thomas Samuel and Mary Ann Blackshaw, of 23, Olive Street, Derby. He was a house painter prior to enlisting 10th August 1914 at Derby. He went to France  11th November 1914 and was killed in action, 9th August 1915, age 27, in the attack on the German positions near Sanctuary Wood, Belgium.
