Battalion: 9th Battalion Sherwood Foresters (Notts & Derbys) Regiment.
Born and lived at Staveley, Chesterfield, Derbyshire. Son of George Haslam and Elizabeth Barlow, of Sycamore House, Staveley. He was a headteacher prior to enlisting at Derby. He went to Gallipoli on 1st July 1915 and died of wounds, 21st August 1915, aged 29.
