Born in Nottingham and lived at 9 Princess Square, Millstone Lane, Nottingham. Son of Michael and Catherine Cusick of 7 Snow Hill, Nottingham. Husband of Jane Cusick. He was a carter prior to enlisting on 13th May 1912 at Nottingham. He went to France on 11th November 1914 and was killed in action, 9th August 1915 in the attack at Sanctuary Wood, near Ypres, Belgium.
