Born and lived in Derby. Son of George and Martha Cook, of 36, Sherwood Street, Derby. He enlisted on 15th March 1911 at Derby and went to France on 4th November 1914. He was killed in action, on 21st December 1914, aged 24, whilst serving in the trenches near Neuve Chapelle, France.
