Born in Nottingham and lived at 10, Burwell Street, Nottingham. Son of Tom and Lucy Cook, of 11, Palin Street, Hyson Green, Nottingham. He was a storekeeper for the corporation electrical department prior to enlisting on 3rd June 1913 at Nottingham. He went to France on 28th February 1915 and was killed in action, 31st July 1915, aged 19, during the German attack on the British line near Sanctuary Wood, Belgium.
