Born and lived at Ockbrook, Derbyshire. Son of Alfred and Elizabeth Cook, of Green Lane Ockbrook. He was a coal miner prior to enlisting on 10th November 1914 at Derby. He went to France on 2nd May 1915 and suffered gunshot wounds to his right elbow on 13th October 1915. He was transferred to England on 22nd November 1915. Discharged physically unfit for active service, on 22nd July 1916 and died 22nd November 1919.

Buried: Ockbrook (All Saints) Churchyard, United Kingdom. In the East part.
