Battalion: 2nd Battalion Sherwood Foresters (Notts & Derbys) Regiment.

Son of William Henry and Mary Anne Cook, of 106, Chell Street, Stanley Grove, Longsight, Manchester. He enlisted at Manchester and was killed in action, 21st March 1918, age 20, during the German offensive near the Vaulx Morchies /Maricourt line.
