Born at Whittington, Derbyshire. Son of Arthur and Annie Cook, of Sheepbridge, Chesterfield, Derbyshire. He lived at Brompton, Chesterfield. He was a coal and iron commercial clerk prior to enlisting at Chesterfield. He was killed in action, 26th September 1917, aged 23, in the attack near Vlamertinghe, near Ypres, Belgium.
