Battalion : 1st Battalion Sherwood Foresters (Notts & Derbys) Regiment.
Born at Walsall, Staffordshire. Son of John and Esther Chadwick, of 22, Hipper St. West, Chesterfield, Derbyshire. He was a labourer prior to enlisting at Chesterfield. He went to France on 16th March 1915 and was killed in action, 17th June 1917, aged 25, during the German shelling of the Battalion Headquarters at Ypres, Belgium.
Buried : Oosttaverne Wood Cemetery, Belgium. Plot 4, Row B, Grave 5.
