Born and lived at Bakewell, Derbyshire. Son of Mr. and Mrs. G. Chadwick, of Bakewell. He was a Quarryman prior to enlisting, 10th October 1914 at Bakewell. He went to France 29th June 1915 and awarded the Good Conduct Badge, 14th November 1916. He died from cardiac failure, 17th February 1917.
