Lived at Chapel-En-Le-Frith, Derbyshire. Son of Richard Evans; Husband of Annie Crook (formerly Evans), of Manchester Road, Chapel-En-Le-Frith. He was a waggoner prior to enlisting, 7th July 1913 at Chapel-En-Le-Frith. He went to France 25th February 1915 and died 4th July 1915, aged 25, at 43rd Field Ambulance from wounds received whilst out on at working party near Maple Copse, Belgium.
